Notice on the Launch of Electronic Health Declaration Form

Starting from 20 November, 2020, all foreign passengers who are flying from India to China will be required to present the Electronic Health Declaration Form for boarding. Foreign passengers can apply for the Electronic Health Declaration Form by logging on the website of http://hrhk.cs.mfa.gov.cn/H5/ via computer or smart phone. Foreign passengers can fill in the information, declare the health status and upload the two negative certificates of nucleic acid test and IgM antibody test, passport page, as well as valid Chinese visa or residence permit, which will then be examined and verified by the Chinese Embassy in India, or the Consulate General in Mumbai or Kolkata. Upon approval, they will receive the Electronic Health Declaration Form, which will be displayed in QR code with the“HDC”mark. Foreign passengers shall present the electronic or printed code, and follow the procedures for inspection by relevant airlines during boarding.

Chinese passengers(including the Hong Kong SAR, the Macao SAR and the Taiwan region) can still apply for the green health code with the“HS”mark via the WeChat Health Code app.
